U.S. Passport Services

If you need passport services, please call the North Dade, South Dade or West Kendall Regional Library or click here to schedule an appointment with a passport application acceptance agent. A separate appointment needs to be made for each applicant. For example, a family of five would need to schedule five appointments. Appointment scheduling is available daily, excluding Sundays. Visit the U.S. Department of State website for a comprehensive list of everything you need to know about applying for your passport.

Urgent Travel Plans

If you are traveling within 14 days or less, apply at a nearby Regional Passport Agency or Center . For more information, please call the National Passport Information Center (NPIC) at 1‑877‑487‑2778.

Photo Requirement

Passport photo service is currently not available at the North Dade, South Dade and West Kendall Regional Libraries. One official passport photo taken within the past six months is required. You must bring the photo with you at the time of your appointment. For additional photo requirements, visit the U.S. Department of State .

Proof of U.S. Citizenship
(any one document, must be original)

• Certified birth certificate
• Naturalization certificate
  (applicants 15 or under must include a certified birth certificate)
• Consular Report of Birth Abroad or Certification of Birth
• Previously issued, undamaged U.S. Passport
• Certificate of Citizenship
  (applicants 15 or under must include a certified birth certificate)

Proof of Identification
(any one document, must be original)

• U.S. Driver’s License
  (if out‑of‑state Driver’s License, additional identification required).
• Current Military ID
• Current Government ID (city, state, or federal)
• Previously issued, undamaged U.S. Passport
• Naturalization Certificate

Name Change

• Bring an original marriage certificate or divorce decree if your name has changed.
